Assessment of the Real Rates of Homicides and Suicides in the Regions of Russia

Abstract

The mortality rates from homicides and suicides, which serve as an indicator of the psychological well-being and the value of life in society, show a steady decline after the 2000s in Russia. However, another block of causes of death, event of undetermined intent shows the same rapid growth and since 2014 already exceeds the cumulative death rate from homicides and suicides. Researchers believe this block of causes is a reservoir of latent homicides and suicides. As a result, actual data on homicides and suicides are underestimated. The situation is exacerbated by the fact that regional practices of coding the causes of death can largely deform the structure of mortality, which is why the ordinary proportional redistribution of event of undetermined intent within the class “External causes of morbidity and mortality” ICD-10 is not enough. The purpose of this paper is to estimate the actual death rates from homicides and suicides using a model region in which the proportion of event of undetermined intent is minimal. Three options for solving this problem are proposed. The suicide rate increased by 30% regardless of the hypothesis. At the same time, the level of homicides and accidents increased most significantly under the third hypothesis, when all cases of events of undetermined intent were redistributed – there was an increase of 30% compared to 15–20% for the first hypothesis. Assessment for regions, depending on the accepted hypothesis of redistribution of events of undetermined intent, showed differences, demonstrating an underestimation of specific causes of death. In some regions, ill-defined causes are used mainly to hide deaths from suicide (for example, Astrakhan, Samara, Sakhalin and Samara regions, as well as Tuva), and in other regions to conceal homicides (Orel and Vologda regions, Bashkortostan).

Постановка проблемы.

2 Блок причин смерти «Повреждения с неопределенными намерениями» (ПНН), наряду с убийствами, самоубийствами и несчастными случаями, является частью класса «Внешние причины заболеваемости и смертности» Международной классификации болезней 10 пересмотра. Уровень смертности от ПНН в России, начиная с 1990-х гг., существенно вырос [Gavrilova et al., 2008; Смертность от внешних причин..., 2017].
3 Если в европейских странах, США и Японии стандартизованный коэффициент смертности (СКС) от ПНН с конца 1960-х гг. в среднем составлял 5 на 100 тыс. чел. у мужчин и 1 на 100 тыс. чел. у женщин, в России сохраняется десятикратное превышение в сравнении с другими странами [Васин, 2015; Смертность от внешних причин..., 2017]. Сейчас эта причина смерти бессменно лидирует среди внешних причин у женщин с 2005 г., а мужчин с 2010 г., когда она обогнала убийства у женщин и самоубийства у мужчин (рис. 1) [Юмагузин, 2017], а с 2014 г. она превышает совокупный уровень смертности от убийств и самоубийств. Исследователи считают эту причину «резервуаром» латентных самоубийств и особенно убийств [Иванова и др., 2013; Andreev et al., 2015]. На протяжении многих лет «минимизация потерь от социально значимых причин путем перевода их в латентную форму» является типичной для России [Семенова и др., 2017]. Поэтому при высоком уровне смертности от ПНН говорить о снижении смертности от самоубийств и убийств преждевременно [Смертность от внешних причин..., 2017; Кваша и др., 2014] (рис. 1).

5 В последние годы в структуре всех внешних причин (ВП) доля насильственных смертей (к которым авторы относят убийства, самоубийства и ПНН) достигла 40–45%; оставшаяся часть приходится на несчастные случаи. Это вынуждает предполагать, что внешние причины смерти (ВПС) не всегда случайны, но все чаще обладают внутренними и внешними мотивами. Более того, различия в кодировании ВПС в российских субъектах сделали региональный анализ смертности по отдельным видам внешних причин малопригодным, поскольку в регионах с высокой долей ПНН смертность от других ВПС будет занижена [Антонова, 2007; Данилова, 2014]. К примеру, в среднем в 2014–2016 гг. в 43 регионах СКС от ПНН у мужчин был выше среднероссийского уровня, у женщин – в 41 регионе, при этом в 11 и 9 регионах у мужчин и женщин соответственно он превышал российский уровень в два раза, а в ряде регионов коды ПНН были проставлены в более чем половине всех случаев смертей от внешних причин (табл. 1). Среднее значение СКС от ПНН среди регионов для мужчин и женщин составили 47 и 11 чел. на 100 тыс. населения, а медианное значение – 47 и 10 соответственно. Стандартное отклонение СКС от ПНН среди регионов – 35 и 9 чел. на 100 тыс. населения у мужчин и женщин соответственно.

8 Высокая доля смертей от ПНН вносит существенную неопределенность в реальную картину смертности. Наряду с другими причинами смерти (например, XVIII класса «Симптомы, признаки...» МКБ-10) этот блок причин представляет так называемые «мусорные коды», потому что они малоинформативны для демографического анализа. Для повышения продолжительности жизни, сбережения здоровья и сокращения людских потерь от преждевременной и предотвратимой смертности важно опираться на достоверную статистику смертности. Поэтому актуальна задача по оценке реального уровня смертности от убийств и самоубийств [Иванова и др., 2013; Андреев, 2016; Семенова и др., 2017; Andreev et al., 2015; Yumaguzin, Vinnik, 2017]. В статье обсуждаются три варианта решения этой задачи на региональном уровне.
