Abstract

The article looks at the state of the U.S. healthcare at the end of the pandemic which brought to light a range of systemic problems of the industry: low efficiency at extremely high cost, inequities to health care access and a strong divide in the American society on healthcare issues. The author examines the key factors that will shape the future of the industry. Among them are an uncertain economic situation and high inflation rates, the rollback of the Medicaid continuous enrollment provisions, the end of the public health emergency and the population aging. The divided Congress also won’t allow the democratic administration to realize large-scaled reforms announced by the President during the race. At the same time some provisions of the Inflation Reduction Act of 2022 will bring some changes to the industry. The analysis allowed the author to come to the following conclusions. American healthcare is experiencing a structural change. On the one hand, the system has to adapt to an endemic future. On the other hand, the difficult economic situation resulting from the COVID-19 pandemic will be another stress test for the industry: shortages of medical personnel, unprofitable medical facilities, rising prices for medical care, and a likely increase of the uninsured. Many traditional players will need to review their operations. Non-traditional retail giants could change game rules and offer more efficient models for managing and health care delivery. Addressing the issue of equitable access to health care will require further intervention by the federal government. Healthcare will be one of the key topics in the 2024 presidential election.

1 Пандемия COVID-19 наглядно проявила многие системные проблемы американского здравоохранения: низкую эффективность при чрезвычайно высокой стоимости, диспропорции в доступе к медицинскому обслуживанию. Как будет развиваться отрасль дальше?

3 Вопросы организации здравоохранения исторически вызывали острую общественную дискуссию и политическую борьбу в США. Это наглядно прослеживается на примере Закона о защите пациентов и доступном здравоохранении (Patient Protection and Affordable Care Act, 2010) – самой крупной реформе в отрасли за последние 50 лет. Ряд обстоятельств позволил принять этот закон. Во-первых, многие положения, включенные в документ, звучали в американском обществе не в первый раз, – часть инициатив разрабатывалась еще во время администрации Б. Клинтона. Демократы сумели извлечь уроки из провала 1993 г. и скорректировать свою стратегию. Во-вторых, итоговый текст законопроекта стал компромиссным. В нем учитывались интересы всех групп влияния: например, страховые компании смогли добиться исключения из него положения о государственном страховании для всех американцев, которое бы конкурировало с частными компаниями, а коалиция работодателей и профсоюзов сохранила корпоративное медицинское страхование, не облагаемое налогом. В-третьих, в 2010 г. обе палаты Конгресса контролировались Демократической партией, что позволило провести законопроект быстро, не затягивая обсуждений [1].
4 С момента подписания закон подвергался постоянной критике со стороны Республиканской партии, которая вела активную кампанию по его отмене. Предвыборная кампания Д. Трампа в 2016 г. была построена в том числе вокруг призыва отменить реформу. Трижды – в 2012, 2015 и 2021 гг. – Верховный суд отклонял иски республиканцев и подтверждал конституционность закона. Все же 10 республиканских штатов не расширили программу Medicaid1, а Д. Трампу удалось отменить положение, обязывающее граждан приобретать страховку или платить штраф за ее отсутствие. Но в целом закон устоял и служит основой политики администрации Дж. Байдена в сфере здравоохранения. Спустя 13 лет тезис об отмене закона ушел даже из повестки Республиканской партии. Таким образом, реформу, проведенную при президенте Б. Обаме (Obamacare), можно считать состоявшейся, хотя она не завершена и требует доработок [2]. 1. Государственная программа вспомоществования малоимущим американцам, обеспечивающая доступ к медицинскому обслуживанию.
5 Двоевластие в Конгресс, ставшее результатом промежуточных выборов 2022 г., не позволило провести сокращение расходов на здравоохранение, о котором заявлял Дж. Байден, выдвигаясь на пост президента. Предполагалось создание общедоступной федеральной программы медицинского страхования, которая составит конкуренцию частным страховым компаниям, и снижение возраста, необходимого для получения государственной страховки Medicare2, c 65 до 60 лет. Со своей стороны, республиканцы намерены инициировать расследование деятельности демократической администрации в период чрезвычайного положения в области здравоохранения в разгар пандемии COVID-19 и получить отчет о триллионных расходах, спровоцировавших инфляцию. Все же в повестке дня администрации Дж. Байдена остается ряд вопросов, которые могут получить двухпартийную поддержку: расширение доступа к услугам телемедицины на бессрочной основе, разработка комплекса мер по борьбе с опиоидным кризисом3, поддержка профилактики психического здоровья. 2. Государственная программа страхования пожилых американцев старше 65 лет и инвалидов.

3. В США сейчас наблюдается рост смертности, вызванный передозировкой опиоидными препаратами, в частности фентанилом. Одной из причин считается чрезмерное назначение болеутоляющих лекарств, вызывающих зависимость (оксикодон, гидрокодон, морфин), для лечения хронических заболеваний. Зависимость и стремление найти более дешевые аналоги привели к росту спроса на нелегальные препараты.
