Abstract

The shortage of medical personnel is becoming a heavy burden on modern healthcare systems around the world. All countries, without exception, turned out to be vulnerable, regardless of the level of income and available forms of medical care. This study addresses the issue of staffing at the time of transition to a new technological level of the healthcare system in the United States. The paper shows that at the present stage in the country there is a need for fundamental changes in the personnel policy in the medical field. The challenges caused by the COVID-19 pandemic not only exposed all the vulnerabilities of the national health system, but also opened up opportunities considering the lessons learned to rethink the long-term vision of solving the workforce problems directly related to the use of new technologies. Based on the analysis of statistical data and sociological research, the author identified the latest trends of overcoming barriers in the field of training and retraining of personnel in the context of the transition to digital medicine. Overexertion among medical personnel associated with increased workload has exacerbated the problem of their professional burnout. The mass protests of physicians necessitated the development of new approaches to the labor protection of employees in the medical field. Solution of such important social problems will require a long time and financial costs and is possible only through the joint efforts of the state and the entire community as a whole.

1	Социальный и экономический потенциалы современного государства, его конкурентные преимущества во многом определяются мерой эффективности национальной системы здравоохранения. В течение последнего десятилетия статистика фиксирует устойчивый рост затрат на здравоохранение практически во всех странах, которые к настоящему времени достигают от 5 до 10% ВВП. В США уровень вложений приближается к 18%, что значительно выше, чем во многих развитых странах, но при этом Соединенные Штаты занимают одно из последних мест в мировом рейтинге эффективности систем здравоохранения [ист. 1]. Существует целый набор глобальных рейтингов здоровья, в которых используются различные данные в зависимости от цели исследования.
2	Индекс агентства Блумберг при оценке эффективности мировых систем здравоохранения учитывает как общий уровень здоровья нации (через показатель продолжительности жизни населения), так и экономическую составляющую термина “здоровье”, принимая во внимание долю совокупных расходов на здравоохранение в структуре расходов ВВП. Значительная доля затрат на оказание качественной медицинской помощи приходится на обеспечение системы здравоохранения медицинским персоналом. В целях углубленного анализа состояния кадрового потенциала национальных систем здравоохранения используются различные методики, разработанные и апробированные в международных организациях (ВОЗ, ОЭСР, Всемирный банк). Появляется такой термин, как “кадровая эффективность”, которая напрямую связывается с показателями общего числа врачей и среднего медицинского персонала с учетом их возраста и гендерного состава [1]. В США проблема воспроизводства кадров для обеспечения слаженной работы медицинской сферы всеми категориями специалистов многие годы была и остается одной из самых сложных и требующих постоянных усилий как со стороны государства, так и всего сообщества в целом.
3	КАДРОВАЯ ЭФФЕКТИВНОСТЬ
4	В последнее десятилетие в медицинском сообществе нарастает озабоченность в связи с усилением тенденции нехватки медицинского персонала во всех регионах мира. В экономически развитых странах эти процессы аналитики связывают с явлением положительным – значительным увеличением продолжительности жизни населения. В результате, в силу старения общества, значительно меняется клиническая картина заболеваний. Спрос на все более расширенный комплекс медицинских услуг растет значительно быстрее, чем воспроизводство медицинского персонала. По оценкам экспертов, при сохранении подобной тенденции дефицит врачей и медицинских сестер во всем мире составит около 10 млн человек к 2030 г. [ист. 2]. Вместе с меняющимися потребностями и общественными запросами, существует настоятельная необходимость в разработке экономически эффективных стратегий, которые снижают затраты на общественное здравоохранение. По данным, представленным в отчете Фонда Семьи Кайзер (KFF), медицинские расходы в США с 2001 по 2020 г. росли значительно быстрее, чем на другие виды услуг и товаров. Увеличиваясь от 1 до 5% в год, они поднялись во время этого периода до 110% по сравнению с 75% роста на другие потребительские товары. Причем значительная часть всех этих расходов традиционно составляют затраты на рабочую силу. Становится очевидной необходимость формирования новой кадровой политики с учетом всех негативных факторов и их воздействия как в краткосрочной, так и долгосрочной перспективе [2].
